Ostojic, Stojanovic et al. 2014 (Journal of Lifestyle Medicine) randomised 22 patients diagnosed with fibromyalgia (ACR 2010 criteria) into two groups: HRW (1 L/day, 7 ppm) vs visually identical still water placebo.
Primary outcome: VAS pain index (0-10 scale). Measurements at T0 and T+4 weeks. Secondary outcomes: Fibromyalgia Impact Questionnaire (FIQ), SF-36 quality of life score.
At T+4 weeks: VAS pain −38% (from 7.2 to 4.5) in the HRW group vs −7% (from 7.1 to 6.6) in placebo (p=0.002). FIQ improved by 21% vs 4%. SF-36 vitality +28% vs +6%.
Modest sample size (pilot study — n=22) but very significant p-value and real clinical effect magnitude. The Acar 2019 meta-analysis confirms the direction and magnitude across ~120 patients cumulated.
