Why male fertility is of interest to H₂ research

Oxidative stress is one of the documented causes of impaired sperm quality: plasma membrane lipid peroxidation, DNA fragmentation, and reduced motility. Approximately 30-80% of male infertility cases exhibit high seminal oxidative stress (Agarwal et al., 2014). Pilot study on athletes with fibromyalgia syndrome. 4 weeks of H₂-enriched water (4 ppm, 720 mL/day). Significant improvement in VO₂ max, decrease in systemic oxidative stress (8-OHdG, MDA). Nakata et al., 2015 — Reproductive Toxicology (animal model)

Rats exposed to cisplatin (chemotherapy-induced). Oral H₂ for 8 weeks. 41% reduction in sperm DNA fragmentation, partial restoration of motility and count. Animal study only, cannot be extrapolated to humans.

Curtasu et al., 2019 — Andrologia (in vitro)

Human sperm incubation in H₂-enriched medium. Reduction in membrane peroxidation markers, preservation of progressive motility at 4 hours. In vitro study; clinical extrapolation premature.

Liu et al., 2021 — Free Radical Biology and Medicine (review)

Narrative review of H₂ effects on reproductive function (animal models). Conclusion: "Preclinical data are promising, but no randomized clinical trials have been conducted on idiopathic male infertility."

Hypothetical Mechanism

Spermatozoa have a plasma membrane rich in polyunsaturated fatty acids—particularly vulnerable to peroxidation by •OH. H₂ crosses the membrane by passive diffusion (low molecular weight, lipophilic), reaches the mitochondrion, and selectively neutralizes •OH without affecting the signaling ROS necessary for capacitation and the acrosome reaction. This mechanism is described in the literature as a research hypothesis; randomized clinical trials still need to be conducted. Results observed in studies or models—not a promise of individual results.
