H2 vs NAC vs glutathione: 3 antioxidants compared

Three molecules dominate the antioxidant conversation among informed biohackers: N-acetylcysteine (NAC), glutathione (oral or liposomal), and molecular hydrogen (H2). All three are described as "the best antioxidant" by their respective promoters. The reality is more nuanced: each acts under a different logic, with distinct bioavailabilities, selectivities and safety profiles.

This comparison draws on two key studies (Ohsawa 2007 in Nature Medicine and Iuchi 2016 in Free Radical Research) to distinguish the specificity of H2, without demonising or overstating NAC or glutathione. Objective: enable an informed user to build their stack knowingly.

Mechanism: 3 antioxidants, 3 distinct logics

N-acetylcysteine (NAC). Direct precursor of intracellular glutathione. Classic oral intake 600 to 1,200 mg/day. NAC crosses the digestive barrier, deacetylates to cysteine, then feeds the synthesis of endogenous glutathione via glutathione synthetase. Documented effect on pulmonary mucus production, hepatic protection in paracetamol overdose, interest in psychiatry for certain OCDs. Selectivity: low. NAC feeds a generic pathway that acts on the whole cellular oxidant pool, without distinguishing useful from deleterious species.

Glutathione (oral or liposomal). Cysteine-glycine-glutamate tripeptide. It is the major physiological intracellular antioxidant. The challenge is bioavailability: the classic oral form is largely hydrolysed in the intestine. Liposomal forms improve measured plasma bioavailability, but actual intracellular penetration remains debated. Selectivity: strong inside cells, but exogenous intake struggles to reach target compartments (brain, mitochondria) in most consumer preparations.

Molecular hydrogen (H2). The smallest molecule in existence. Diffuses freely across any biological membrane, including the blood-brain barrier and inner mitochondrial membranes. Bioavailability essentially immediate after ingestion of hydrogen water. Selectivity: this is the point identified by Ohsawa et al. (2007) in the founding publication. H2 reacts preferentially with the hydroxyl radical (•OH) and peroxynitrite (ONOO) — the two oxidative species most deleterious to proteins, lipids and DNA — without neutralising useful signalling species such as H2O2 or physiological NO.

This selectivity distinguishes H2 from NAC and exogenous glutathione. Iuchi et al. (2016) confirmed in vivo that H2 does not disrupt normal redox-signalling pathways, unlike high intakes of generic antioxidants that can blunt certain exercise adaptations (effect described by Ristow for vitamin C/E at high doses).

Protocol: can the three be combined?

Limits and nuances

This comparison describes mechanisms and user observations. None of the three molecules is a drug in the strict sense (NAC is used pharmacologically in very targeted indications, but its preventive daily intake belongs to dietary supplements). Clinical evidence on hard markers (mortality, cancer incidence) remains absent or limited for all three.

NAC can interact with some treatments (nitrates, high-dose anticoagulants). Glutathione can interfere with some chemotherapies (oncologist's opinion mandatory). H2 has no documented drug interaction to date, but this lack of signal does not imply lack of effect.

H2 audience not recommended. No documented absolute contraindication. Pregnant or breastfeeding women: as a precaution, validate use with the treating physician. Children: insufficient data, to be avoided.
