H2 and autophagy: mTOR / AMPK mechanisms explained

Autophagy has become the Holy Grail of longevity hackers. Spermidine, rapamycin, intermittent fasting — everyone is trying to activate this cellular recycling mechanism that may slow ageing. Molecular hydrogen (H2) enters this equation through an elegant pathway: it simultaneously modulates the two master regulators of autophagy, mTOR and AMPK, without requiring extreme caloric restriction.
This article decodes the molecular mechanisms identified in three key publications (Hancock 2024, Mariani 2025, Ostojic 2018), proposes an experimental protocol combining H2 and a 12-hour fasting window, and explores stacking pathways with rapamycin and NMN. The whole, framed by indispensable caution recommendations on this still-emerging scientific terrain.

Mechanism: how H2 modulates mTOR and AMPK
Autophagy is governed by an antagonistic balance between two kinases. mTORC1 acts as a nutrient-abundance sensor: when active, it inhibits autophagy and promotes protein synthesis. AMPK is the energy-stress sensor: active when the AMP/ATP ratio rises, it triggers autophagy to recycle damaged cellular components. To activate autophagy, mTOR must therefore be lowered and AMPK raised.
Ostojic's work published in Medical Hypotheses in 2018 laid down the first mechanistic hypothesis: H2 would act as a weak mitochondrial signal, activating SIRT1 and AMPK via redox potential modulation without inducing oxidative damage. This "hormetic" signalling reproduces some effects of caloric restriction without its constraints.
Hancock and his team (2024, Antioxidants) confirmed in vitro that cellular exposure to dissolved H2 significantly increases AMPK (Thr172) phosphorylation and reduces mTOR (Ser2448) phosphorylation after 60 minutes. Autophagic markers LC3-II and p62 evolve in the expected direction of active autophagy. The effect is observed at concentrations physiologically achievable by ingestion of H2-enriched water.
Mariani et al. (2025, Free Radical Research) extended these observations to an animal model: 8 weeks of H2 supplementation in aged mice raises hepatic and muscle autophagy markers by 28 to 41%, without loss of lean mass. The AMPK signal remains elevated even far from ingestion, suggesting a durable adaptation.
Protocol: H2 + 12-hour window, dosing and timing
The protocol below comes from the practice of advanced quantified-self HYDROGENYX users. It combines a classic nocturnal fasting window (12 h) and three targeted H2 intakes. The objective is to extend the AMPK activation window without inducing the fatigue of a 16/8 or 18/6 fast.

Limits and contraindications
The mechanistic data is solid, but human clinical trials remain rare. Most publications are in vitro or animal. Effects on human lifespan are not demonstrated.
Cautions for stacking with rapamycin. No direct interaction study published. Pharmacological logic suggests a potential synergy (both target mTOR), hence vigilance regarding rapamycin's known effects: dyslipidaemia, glucose intolerance, immunosuppression. Mandatory medical follow-up.
Cautions for stacking with NMN. NMN raises NAD+ and activates SIRT1; H2 activates AMPK. The shared pathway is coherent, with no known antagonism, but combined data are missing.
Audience not recommended. Anyone on immunosuppressive treatment, active chemotherapy, or with a history of flaring auto-immune disease. Autophagy is a complex mechanism: "over-activating" it is not systematically beneficial.
